Step-by-Step Guide on How to Generate an e-affidavit for Change of Name, Loss of SIM, Declaration of Age and General Declarations

The Lagos State Judiciary has taken a significant leap in judicial innovation with the launch of a new electronic affidavit (e-affidavit) system, allowing individuals to file sworn statements online without visiting court premises. This digital solution is part of the broader Lagos Court Management Information System (Lagos CoMiS), aimed at streamlining court operations and enhancing access to justice through technology.
According to a statement by Kayode Agbedejobi, Esq., AICMC, Publicity Secretary of the Nigerian Bar Association (NBA), Ikeja Branch, the new e-affidavit process became mandatory from June 1, 2025, for all general affidavits such as:
- Change of Name
- Loss of SIM
- Declaration of Age
- General Declarations
These affidavits must now be created exclusively through the Lagos CoMiS platform:

It is important to note that affidavits for High Court litigation purposes are exempt from this directive and will continue under the existing manual process.
For micro, small, and medium enterprises (MSMEs), startups, and individuals frequently needing affidavits for business registration, official declarations, or compliance requirements, this system eliminates long queues and manual paperwork, allowing for efficient, traceable, and legally valid documentation, all from a mobile phone or laptop.

- Online Creation, Signing & Notarization: All affidavits generated are legally compliant and handled by authorized judiciary personnel.
- Secure Payment Integration: Court fees can be paid securely via debit/credit cards or alternative channels.
- Remote Authentication: Deponents can upload video identification and e-signatures from any location.
- Digital Recordkeeping: Users can preview, save, and update affidavit entries digitally.
How to Create Your E-Affidavit on Lagos CoMiS
Here’s a step-by-step guide:
- Visit the Platform
Go to https://lagoscomis.lagosjudiciary.gov.ng - Log In
Enter your legal email address and password, then click “Sign in.” - Start a New Filing
On your dashboard, click “New Filings” at the top right of your screen. - Select Affidavit Type
Choose from available affidavit types or select “General Affidavit” for custom needs. - Fill Out Form
Input accurate details, upload a passport photograph, and click “Create Affidavit.”- Edit paragraphs by clicking the three-dot icon.
- Add more info using “Paragraph,” “Sub Paragraph,” or “Sub Sub Paragraph.”
- Be sure to save after each input.
- Preview and Finalize
Click “Preview and Proceed”- If there’s an error, click “Cancel” to edit.
- If all is correct, click “Done.”
- Complete Your Profile
Enter your full name, email address, and phone number. - Make Payment
Click “Proceed to Payment” to pay the required court fee. - Upload Verification Assets
Upload a short video ID of the deponent and the digital signature. - Guidelines and detailed instructions are available on the platform’s homepage.
